VGIF Grant Application Requisition Form

Please read the VGIF Project Grant Application Guidelines before proceeding.

Please read the instructions before filling in the form below.

1. Fill in the form below including the required fields. You must have a valid e-mail address to download the application form.
2. Once you submit the information requested, you will be sent to a Thank You Page where you will be able to download the application form as an MS WORD document. You will also receive an e-mail message of confirmation that you should keep in your records.
3. Once downloaded, save the document on your hard drive, fill it out and include the APPLICATION NUMBER (that was assigned to you) on page 1. Then send the completed application via e-mail attachment to vgif@vgif.org.
4. Please use the assigned APPLICATION NUMBER on all future correspondence with VGIF and on the first page of the application form.
5. If you would prefer to receive a hard copy application form or an e-mail attachment, please contact vgif@vgif.org
